Major Medical Insurance
Please note that I am NOT in network with any major medical insurance.
When you get services from an out-of-network provider, you're responsible for the difference between what your insurance pays & what the provider charges. You will pay upfront & I will provide you with the appropriate documentation for you to send to your insurance for reimbursement. Reimbursement is only paid at the insurer's allowable rate & if you have out-of-network benefits.

Seeing an out-of-network provider might cost more out-of-pocket as far as insurance, but you still end up paying less than the providers regular rate. Most plans, massage is not subject to the deductible. You are responsible for knowing your out-of-network benefits and what paperwork is needed for reimbursement.
Labor and Industries / Worker's Comp
on the job injury
If you have obtained an on the job injury and have an active, open claim L&I may pay for treatment massage. In addition to an open claim, you will need to have a prescription/referral from your PCP, chiropractor or physical therapist.
Please bring to your first appointment your case number, PCP or chiropractor prescription with diagnosis codes, and claim adjuster’s information.
Personal Injury Protection (PIP)
Auto Insurance (auto accident)
In the event that you have been involved in an auto accident and acquired injuries, your auto insurance may cover medical bills if you have Personal Injury Protection (PIP) coverage. It is recommended to have an attorney.
You will need a prescription/referral from your PCP, chiropractor, or physical therapist along with an open accident claim, your claim and adjuster information, and attorney information. Your referral will need to have diagnosis codes and number of treatments.
At this time 3rd party claims will not be accepted.
